I recommend you prepare the draft on your own and upload it on this forum. This will help improve your drafting skills. If you get a readymade draft, it will be spoon-feeding. This spoon-feeding could hold back your growth. Secondly, just because someone helps you does not mean that the draft is error-free. I have seen mistakes in the drafts provided by fellow members. However, rather than correcting the draft prepared by the originator of the post, it becomes too embarrassing to correct a fellow member's draft. Anyway, without practice, one cannot attain perfection.

Just send him a letter which outlines what his offences are, e.g incorrect attendance procedure, and the provable misbehaviour within the team.
Then tell him what the penalties are, and what action management proposes to take if he ignores this warning.
Never tell people they are doing something wrong WITHOUT telling them what the penalties are if they persist in their actions.
This is a simple letter, and well within your capabilities as a HR person. You have no problems telling us, so tell him the same thing.
