Dear Friends, Is there any declaration form available with you about employees declaring details about their relatives who are being employed by the same company? Please share if you have one.

This is normally being done during recruitment, while screening of application process. This information normally being used, based on the policy of the organization ‘not to entertain / recruit employee’s relatives’, anticipating sharing of confidential information / and interfering disciplinary process / concerted actions within the organization against the Management or Management Personnel.

Some old establishment, this procedure must have been overlooked over years, due to lack of policy documentation.

If you want to collect such information, now, better to notify in the ‘notice board’ and request all employees, to declare, in writing about their relatives been employed in the establishment or the allied group establishments.

You have asked for a form for the employees through which they can give a declaration about the employment of their relatives in the company. However, why do you wish to have this declaration? You have not mentioned it.

Mere obtaining the declaration is not sufficient. You need to go further and design a "Policy on Conflict of Interest".

In the legal field, many times lawyers have their lawyers and judges are related. In case, if a case is handled by a lawyer and is due to be heard by a judge who happens to be a relative of the lawyer, then the judge recuses from the hearing to avoid the "conflict of interest".

Therefore, whether you obtain a declaration or not, you need to design a policy on conflict of interest.
