Dear Madam,
For disciplinary action, you are governed by the Industrial Employment Standing orders Act , if applicable, or company's own Service rules.
For minor misbehaviors, and committed for the first or second time , counselling and mild caution works effectively. But for serious misconducts which mentioned in the first para, you need to issue a show cause notice giving full facts of the incident like date, time place and sequence of events as happened and requiring him to to submit written explanation within forty eight hours.
If no explanation received or explanation is found unsatisfactory, proceed to issue the charge sheet and later conduct the domestic enquiry. Based on the findings of the enquiry report, if charges are proved then award appropriate punishment considering gravity of the misconduct and past record as per the Model Standing Orders / Service Rules. It could be, warning, suspension without wages or discharge/dismissal.
