If we are changing a few terms of employment which includes a few changes in salary also so it necessary to issue entirely new appointment letter or we can issue addendum.

If you issue an amendment letter quoting the para ibid that should serve the purpose. Issue a fresh letter with revised terms might pose certain problems, like, date of letter and its effective date, acceptance etc.
With regard to salary revision, it doesn't require to amend the apptt.letter itself. To this effect just issuing a separate letter would suffice.
