Hello sir/mam,
I want to issue a show cause notice to one of our employee who is not achieving his target since last 3 months and also because of his continuous absenteeism. Can you plz help me to draft the same.

Dear member,
We the members of this forum are outsiders. We do not know the facts of the case completely. Show cause notices cannot be vague. We need to quote actual facts. Therefore, I recommend you preparing the draft on your own and uploading it here. Some senior like me will correct it.
Lastly, about maintaining anonymity while seeking help from others. Your post does not withholding your identity. Issue of show cause notice to an errant employee is a routine HR matter. Therefore, when you upload your next post, there is no need to make anonymous.

Today you have availed leave by informing over phone. It has been observed in last three times that you availed leave due to sickness or domestic issues over phone. You have recently availed annual leave from ....... to ......
This was despite clear instructions by CEO and Director that no one should combine leave along with Saturdays.
Your presence was absolutely important to complete the job entrusted to you of .....client name. Your casual approach and unauthorised above leave has created problem in meeting commitments to our clients.
We consider this as wilful disobedience and therefore are constrained to issue this show cause notice for explaining the reason for unauthorised absence and disobedience.
You can submit your explanation in writing within 3 working days form the receipt of this notice.
