Hi,I want send a warning letter to a staff for using inappropriate or bad language with his co workers at work place, can someone share a sample?

Dear Napean,
We the members of this forum are outsiders and do not know incident that has happened in your company. Therefore, you may write draft of the warning letter on your own and upload on this forum. Some senior like me will correct it.
You may use the following guidelines to make a draft:
a) Write background information, who was on duty, where they were on duty and what interaction took place
b) What was the severity of the unparliamentary language, who used it first? Was it part of retaliation?
c) Then write about importance of workplace etiquettes and how it impacts the culture of the company
d) Lastly come to the warning part. Write that in case if the person commits same mistake again, it will be viewed seriously.
