Dear Rushaan,
Following could be the draft of your email:

Payment of Sales Incentive
Dear Sir,
As you are aware, I am working as _________ (your designation) in our company since __________ (date).
For the last financial year, my sales target was _____ whereas I achieved sales of ______. When compared with my peers, my achievement puts me at the top of the list of the persons who are eligible for the sales incentive. Somehow my name did not figure in the list and I did not get any incentive so far.
In the month of ______, I had brought exclusion of my name to your kind notice. You had assured that the matter will be looked into. Nevertheless, the matter is pending till date.
In view of the above, may I request you to kindly refer the criteria for the eligibility of the sales incentive once again and recommend my name? As per my calculation, I am eligible to get ________ (mention amount). Since the matter is outstanding for long, my second request to take up the matter for the expeditious disbursement of the incentives.
Regards,
___________ (complete name)

All the best!
