Dear Shakuntala,
first login with the employer user id and password, then click print counter foil, enter the insurance id no of the employee to whom you want to issue form 7B, click view. a page opens up, click view counter foil towards the right side, you will get his temporary identity certificate page. i that page towards the upper left side you will have a link Medical Acceptance Card [Click Here to View Medical Acceptance Card] click on it that form is called form 7B
