Have you click photo with ESIC OFFICE? please ask to your hr person he will give you form temporary id card form .visit ESIC office.check the address of that duplicat card

Sir, please clarify whether at the time of your joining whether you have filled up necessary form called Declaration Form and also whether your employer has handed over you TIC indicating therein your ESIC Insurance Number. If not, then you may confirm the position from your HR department or the concerned officer who is attending to ESIC work in your establishment.
