One of our employee's DOb is wrongly mentioned in EPF and In ESIC it is correct. He has 10th certificate as a proof.

Dear Greeting from the day.
you have to write a letter to concern dept. stating that its was clerical mistake and actual D.O.B. of employee is ........( as actual and display in ID proof also) along with the ID proof of employee. they will change it in their own record.

Kindly ensure that you keep the acknowledgement copy of your submission for future reference because I have seen numerous cases wherein the corrections in DOB or Name have not got reflected inspite of several requests made to the PFO. The best procedure is to do the follow-up every 15 days after the submission date. And if it does not happen, walk up to the PFC or PF Asst. Commisioner (responsible for your region) and meet with him to discuss the issue. This process really works, and on the other hand, you get to know your PFC or PF Asst. C under whom your establishment fall in to, helps to built rapport.

As per ur query i will suggest if there is any clerical error has been done from ur side need not to worry you simply Fill up form-2 (Nomination & Declaration ) form of the respective employee with necessary amendment with covering note also enclose the relevant document in support of the same and submit to the concerned EPFO at earliest. Kept acknowledge copy with you for the reference.All necessary updation & amendment should be done from EPFO side.
