Dear Lingabala,
If you enrolled in ESI you may get the ESI number from your employer and you wil get temporary ID card once you register and your employer can give the dispensary which is nearest for you.. so its not necessary to go your Head office and the ESI dispensary which is nearest to your place is mandatory to get the approval from them for permanent ESI card. you alone go with your proof and speak with ESI officer. they will guide you.
ESI head office in Chennai located in nungambakkam. you may get more info from those people : 04428306300
hope its helpful and you can revert further.

If your company is delhi based and you are working in their chennai Office then , your branch must have got ESI sub code. Based of the Subcode Number, you can get ESI number and family card also. So , you are not required to visit Delhi office from Chennai. Also if your branch office has no subcode number tnen it is totally illegal to deduct ESI contribution from your salary. Please enquire whether they have sub code number or not.

Dear Linga bala,
You can go to any ESI branch Office at Chennai with Temporary Identity Slip and take photo with your family. The T.C. will duly signed by your employer in the photo. Then the card will delivered to your ESI office and you will get it.
