Hi Rachana
You send a mail to my ID so that I can share few of the templates. Apart from that if at all you wanna buy any forms then if you approach the shop Industrial & Commercial Prints in Gandhi Nagar near majestic you will get all the forms & booklets.
Regarding the group health insurance policy I have only limited knowledge. It is better you get from other experts

Dear Rachana
First you take out a list of employees who are not covered under ESIC. Medical health insurance apllicable for employees who are not covered under ESIC only. Then contact any general insurance company for further details. Kindly make a note that your medical health policy should be equal or more than ESIC benifits.

Madam,
Once you collect the quotes from the various Insurance Companies,it is good to tailor-make a policy according to the preferences of the members of staff, more specifically their average age. The minimum cover would be around Rs.150000. But you can allow the cover amount to be decided by the members. Better to go in for nationalized service provider having good number of impaneled hospitals.
