Hi,
If an employee completed his 6 months then Form 19, 10C needs to fill. If he hasn't completed 6months then only Form 19 needs to fill.
The form should have all the details with the authorized signatory and seal from the company.Form 3 A needs to attach which comes every april i.e annual return. Then form needs to be submitted in PF office . Cancel cheque with the address proof should be attach with the form. Address proof needs to sign by the employee. The employee will get the PF within 1 month of the submition. PF form can only be submitted after 2 months once the employee left the organisation.
For futher query, Please mention the issues.

I have a question that I need to know the required action to be taken in case my PF details is not provided even PF was deducted . I have mailed to the accounts for the same some months ago but they are always giving new timelines. I had word with HR as well and it was informed that it will be resolved at the earliest but nothing positive again with new time lines. Ended up with their statement that PF transfer from your's did not happened so they are not able to take further steps but I was having proof of the same from last(previous to that one) organization and I updated them. Again they gave new timelines. I dont think this will take long time if its properly done. I mean I can understand upto couple of months but its almost 6 months/.
Please let me know what action I have to take now.
