Good Evening,
I need help about EPF related issue. I was worked in BPO organization and the organization has deducted EPF amount from salary. I lwas left the organization on Sep'11 and without giving because some personal reason. I was request them to provide me EPF details and where do I need to submit application form. But they didn't provide details about EPF procedure.
Requesting to pls help me out in this regard.

You have to consult with your HR Coordinators constantly. Take followups regularly. Upon receiving of your EPF documents, form 19 & 10 C authorised by your last employer, along with form 3A & 5/10.

thats fine
fill up form no 19 / 10C , Form 5/10, attatch xerox copy of bank statement
request the BPO authorised official to attest the forms
if BPO is not signing the form, take attestation on the forms 19/10C, Form 5/10 xerox copy of bank passbook from bank manager of the bank in which you have a/c and submit it to Provident fund office where the account of BPO is maintained.

hi safiqulsh you can download the form 5/10 from epfindia.com download section, return, monthly 5/10 regards

there are no form 5/10 in download which is relevant to the above case.Form 5 is relevant for deceased person only.

F 5 & 10 are not by employees But by employers earlier But is now dispensed with due to online system.Nominees of deceased employees should submit form 5 under EDLI scheme,not f orm 5 u/pf sheme.
