You'll need to select a 360 instrument and then follow the vendors instructions.
For a state of the art 360 click on "Checkpoint 360 Competency Feedback System".

Being an analyst I would suggest you to prepare a plan of activity - all the tasks to be performed before automation
1. Prepare a policy document on 360 appraisal system
a) objectives
b) rationale
c) whom to be covered
d) roles in process
e) forms in use
f) frequency of appraisal
g)exceptions if any
h) ratings/scale to be used
i) whether to be linked with payroll or any other subsystems
2. Get the approval of all stakeholders before releasing the policy
3. Get the approval of Corporate HR Head to roll out the process
4. If you are planning to automate the process, get permission for that too and allocation of budget
Note:- If you are having more than 50 people in your organisation, it would be very difficult to manage the
process in the absence of an automated system.
