I couldn\'t attend my duties for the past 2 weeks due to health issues. I couldn\'t communicate to this to my boss in detail. I want to write a letter asking him to understand and forgive me for this delay and want to assure him about the performance in future.

Further in this respect, please find attached herewith my medical reference slips and bills----------------------------which shows that for the last two weeks I have been hospitalised or on medical rest as recommended by the doctors.

Sir, it is my mistake that I could not inform you thru email for which I apologie.

In view of the above you are therefore requested to condole my mistake. I assure you that in future this type of mistake will not be repeated and I will prove myself to be a good and productive asset to the organisation.

Overall your draft is ok but a slight revision is required. It is as below:
a) The very first sentence is compound. Split it into two small sentences.
b) We do not know whether Narotham was hospitalised. Therefore, just it is enough if we write "I have attached certificate from the doctors stating I was under medication".
c) In the third paragraph, it is enough if we write "I regret my inability to intimate you through written communication". By the way, is it mandatory to provide written communication in spite of sickness?
d) In the last paragraph, the it is not "condole" but "condone". Secondly, if the sickness was genuine then the wording of this paragraph is too submissive. It is enough if we write "In view of the above, I request you regularise my absence by granting me sick/earned leave. I will perform my duties to the best of my capacities once I attain complete medical fitness".
