Can anyone help me in making a questionnaire for finding out the following:-
1. challenges faced by patients in the hospital.
2. Finding out the problem areas in the hospitals by talking to its employees.
3. Questionnaire for the management.

Dear Richa

I think its your summer training topic. For the quesionnaire, here are types of questions you can include:

CHALLENGES FACED BY PATIENTS:

1. Do you find taking appointment and further your visit as per time allotted to you are in organised way?

2. Hospitality by the staff for the patient

3. Tests, report collection, etc- are they in organised form or you feel harassed?

TO FIND PROBLEM AREAS IN HOSPITALS BY TALKING WITH THEIR EMPLOYEES:

1. What are the problem areas, patients complain for?

2. Is work allotted to you is manageable, or you find yourself burdened?

3. Do you find policies for you are ok? Or you find need of some alterations?

4. Along with this, try to put few blanks for suggestions. As such points may be beneficial for the betterment or improvement if required and significant suggestions if found and addressed would help avoiding differences in management & employees and contribute to the growth and better services by the hospital

FOR MANAGEMENT:

1. Ask if the goals they had set (short and long term), are they achieving it smoothly or facing challenges

2. Do they think team is doing job efficiently as per their expectations?

3. are the doing performance appraisals of the employees and addressing their training needs?
