1. Prescribed form for Allotment of Business Number (BN), duly filled & signed.
2. Copy of our Service Tax Registration Certificate issued by the Office of Assistant Commissioner – Customs, Central Excise & Service Tax, - IF APPLICABLE.
3. Board’s Resolution.
4. Memorandum & Articles of Association.
5. List of Employees as on date.
6. Form No. 9 (Revised) – Detailing the List of employees covered under EPF Act.
7. Form No. 5 A – Return of Ownership
Wageregister,attendance register.
