4. Form 15 - Register of Annual Leave with wages

5. Form 12 - Adult Register

6. Form 26 - Register of Accidents causing Death or Bodily Injury

7. Form 18 - Notice of Accident for Fatal Accidents

8. Form 18(1) - Notice of Accident for Non -Fatal Accidents

9. Form 5 - Certificate of Fitness issued by Certifying Surgeon after Medical Examination of employees.

10.Form 8 - Certificate issued by competent Person for Pressure Vessels/ CPB's

11.Form 33 -Certificate issued by competent person for Lifting Tools/Tackles/Hoists etc.,

12. Common Annual Return (Form 22).

2. Contract Labour (R & A) Act - 1970 : 1. To obtain Registration Certificate.

Notice submitted to employer by employee for payment of Gratuity

Intimation sent by employer to employee to receive Payment of Gratuity

7.Industrial Dispute Act - 1947 : Form P & PA - For Retrenchment

Form Q & QA - For Closure.

8. Indl.Employment(Standing Orders) Act - 1947 : Submission of Draft copies of Standing Orders for certification.
