One of my employee while he was coming to office,accident occurred and dead.
He is in ESIC.what type of documents will submit in ESIC and PF.
Pls share your Valuable Message.

Hi Venkat,
Just prepare an accident report in ESI Accident notification and attach a copy of the FIR, the Hospital Treatment Report and the Funeral certificate. For any further help contact your local ESIC office.
For PF, his/her nominee can raise claim as usual in the event of normal death.

Hello venkat,
As per esic , the nominee will get Rs 5000/- as funeral expenses only.
in case of epf, the nominee and the minor dependents will get the monthly pension and for that you need to submit the required documents like 10-D, Form 5-IF etc.

Dear Venkat,
You have also to fill in the form for Funeral benefit. The amount is Rs. 10,000 which the family of deceased will receive from ESIC. Further, his family will also receive Rs. 1,30,000/- from EDLI as you pay contribution in his PF.

regarding esi .form-16 its a accident report tat should be submitted to chief inspector of factories and Another to the insured medical officier concerned.
Form-25A for claiming funeral expenses to be submitted with death certificate /declarations duly attested within 3 months from The date of death

Dear, clement, I think EDLI benefit is not fixed at 1,30,000/- but it depends upon the service and epf deposited on behalf of that particular employee.

I agree with all the suggestions given by you but still I am having some questions in my mind regarding the same
1. The employee coming to office should be in company transport for getting all the benefits or not ?? If he is coming by some other mode then he will get all the benefits. The coverage of ESIC is only at working premises or every time & anywhere ???
2. Accident report would go to both ESIC office and factory inspector ??
3. 5,000 as funeral expenses will be given from ESIC but 10,000 is for wat ? I dont think so that EDLI is fixed as said above 1,30,000
Please clarify my doubts............

Pl.go thro' the attached extract from ESI Act and note the highlighted portions for your guidance. But there umpteen no. of case laws on the subject very precisely w.r.t. the circumstances which you referred here. You better refer them also to have a clear understanding of the context and guidance. If you don't get them let me know.

Reg. PF there are few procedures which already indicated by many.

Reg. other benefits etc. better refer the Wokmen's Compensation Act also which will be added advantage.

First and foremost are- i) Police FIR certified copy, 2) post-martem report, 3) death certificate issued by the local administration 4) Collect copies of nominations filed by the deceased under/for ESI, PF, Gratuity Act etc.

5) File papers for obtaining 'Succession Certificate' in favour of the deceased's nominee/wife as the case may be at the appropriate court.

6) In order to smoothen the process obtain " NOC " from the close relatives such as parents, brothers, sisters or any other dependents(if any).

Funeral Expense enhanced from Rs.5,000 to Rs.10,000 w.e.f. 1.4.2011 vide notification No.R.15/13/2000-Bft.II dt.19.5.11 issued by the Joint Director (BFT), ESI, New Delhi.
