ESIC form 16 is the accident report which is to be filled within 24 hours of the accident. It is only filled for those employees who got injured during work hours and not otherwise.
Form 16 includes all information from the occurring of the accident and causes to the witness of the accident.
Form 12 is a sick leave remuneration application.

Pregnancy Claim can not be raised by the Company. It has to be done on the part of the employee himself or herself.
Now Under ESI: 86 day salary is given to the pregnant woman ( if she is on leave on account of pregnancy and if she is the member not member's wife) and all the expenses during the birth till leave from the hospital.
