Create your competence dictionary - look at the position while defing the competencies ( tech + behavioural) , you can also have BEI with the best performers & take feedback from managers for the same.
On the basis of that ask the incubants for self rating, with example for indicators of that competency ( if required)

The steps in the process of Competency Mapping Process are outlined in the appended document.
I fail to understand what questionnaire / s people are referring to? If you don't know the process, first study it from a reliable source.
