I am mentioning below some points which can be used as a guideline for the purpose of creating a policy:
1) Define the purpose of the policy
2) Decide which section of employees is to be benefitted by the policy.
3) Define the basis (conditions which are to be fulfilled) on which amounts will be payable.
4) Describe the process for claiming benefit under the policy, mention any special documents which will be required as proof.
5) Define the time frame and mode of payment (timeframe= monthly/weekly/quarterly etc).
6) Specify (as exhaustively as possible) any exceptions to the policy. This may include categories of employees who may not necessarily be covered or excluded from the policy.
7) In the current financial scenario, you should mention the taxability of such benefits.
8) Create and attach any formats required for claiming the benefit.
If anyone else can add to this, please do.

Sorry Bips,
I dont have any kind of survey data which I can share.
What I can suggest is that you make you policies as tax friendly as possible. eg. allow medical reimbursement upto 15000/- p.a. and ensure that this is paid as a reimbursement against proper medical bills. Or perhaps you can look at introducing Food Coupons.
