I want to conduct employee satisfaction survey in our company.
The main aim is to find the reason for increasing attrition rate in my company.
Its an IT company of 50 employees. Considering the growth in market we are giving good hikes during appraisals but even then attrition rate is increasing. So, my management wants to know the reasons behind this so that the problems can be solved.
Please help me in making employee satisfaction survey for the above reason.

Thanks Avika and Chitra . An employee satisfaction survey should be done by a third party and not the in-house HR. Even if the process is ran by the HR Team , it still needs to be audited by the external team.

The challenge with your company is that it falls within SME category. Yours is not even medium but small type company. The reasons of attrition are within your post as well. Dear friend Keerat, try to understand, people don't work for money alone. By giving good hike, you cannot buy people. What people expect is good working environment. Good environment is not just physical environment but psychological environment as well. Attrition will continue to happen as long as there is no:
a) culture of empowerment
b) culture of inclusion (each of every member must feel that he/she is part of decision making process).
c) culture of learning (this will come provided higher management spends 'x' hours every years in learning).
d) culture of change
e) culture of collaboration
f) culture of self-discipline
g) culture of competition
h) culture of recognition
i) culture of innovation
I am of the opinion that in real sense you may not require ESS. In most of the vernacular languages of India, there is a parallel proverb to the Hindi proverb "Haath ke kangan ko aarsi kyon?". ESS is something like that.
