Helo sir i know about esic smart card. My one employe have a smart card by his prevous company but he left this form and our group so plz conform me what i do

contact your area inpector and local office of esic and submit the new df bimetric form of employee than department arrange the camp for preparation samrt card
this card valid in all over india in pannel of esic and all dispecsary
these are two saprate card one for family and one for employees
may be in futurte it is very usefull for insured person
as i heared that from aug 2010 esic dispesary only accept the samart caard for medical benefit

After computerisation of ESI Branch Offices w.e.f. 1.8.10, certain details such as Name of the employer, Address of the I.P. etc., can be changed by the concerned Branch manager through online.. Further i feel that ur question is not clear.........SYAM, ESI INSPECTOR

I cud not go when esi camp had been conducted in our co. for ESI smart card.
Now, how can i apply for the same, i've my ESI no. also? Can i directly apply or i will have to wait when will our co. conduct this camp again in future?

AS I understood one card is for employee and another for his family memebers/dependent. for family memebers card I group photograph has to be attached with form or all family members should to to esic office for taking photo. Please clear.

No all ur versions will be wrong. I will clarify the doubts one by one:

01. If a person fails to attend the Photo Camp: No need to worry, again the camp will be conducted in your area, till that time u have to wait. Nothing to worry, though it was fully computerized till it was streamlined fully, even manual cards will also been accepted for treatment. Further as an initial stage WIPRO people are conducting the Photo Camp, in the later stages, the concerned Branch Office will only do this work;

02. In case of addition of family members: Though at the first instance the employee was provided with his single photo only. The cards provided at the first time are the permanent cards, no question of adding the other members by providing the photos along with any Form. But as i told in the later stage the same will be done at the Branch Office level, whomever they want to add they have to bring them to the local office along with the smart card, the concerned Branch manager will add the particulars of additional family members in the back end data of the software.....

Don't worry about the ESI Smart Card. Once u are covered under ESI and enrolled under ESI by submitting the Declaration Form to the concerned Branch Office through your employer, that is the duty of the ESIC to provide the smart card to u. . U just enquire the concerned Branch Office about the probable date for photo session in your area for smart card..............

Dear gusha.
Don't break ur head regarding the Phot camp. Primarily ESIC is taking it as its primary responsibility to provide the smart card to all the eligible insured persons.
Regarding the details of photo session, kindly enquire the tentative dates with ur ESI Local Office Manager........
Till the computerisation get its pace, even the manual cards will also been entertained........... But it always advised to register under Smart Card at the earliest.........
