this is aisha ahmad and i urgently need your help concerning my thesis....actually i m doin my thesis on a unit of textile industry...and i have to identify a problem, forming hypothesis, and then use SPSS to analyze my findings...i have identified one problem and that is problem in grievance handling.....now i have to make a questionnare.....can you please send me a questionnaire that is employee oriented...means in which the questions are asked by the management. my hypothesis are
1. company does not have a proper group of HR advisors
2. employees lack confidence in disclosing their grievances
3. ineffective communication between management and emplyees
4. centralized desicion making in the copmany that creates frustration and delays
5.poor HR practices

Have you done any literature survey? if it is for your final project, then you certainly have to survey work by others, form your conceptual framework and the frame your questions based on the framework elements that should link with your objectives/hypotheses. Please see my posts on Research methodology and MBA Disseration.
