You can ask your team leaders to speak with their teams and give you the names of employees in a format of red green and yellow. Red--people with reasons that can not be controlled like marriage, relocation, further studies etc.
Yellow--people with reasons that can be controlled like performance, growth opportunity etc.
Green--people who wants to stay with the organisation and will not leave the organisation.
As per my observation an employee never leave a company he only leave a bad supervision. so the supervisors are the only one who can tell you the pulse of their team. you can then sit with all the employees with red and yellow band and work out the best possible solution for the.
If you want to have more information about the factors that push the employees to leave the organisation then please chek cit hr or google.

Yes for this you have to design a questionnaire for departments. That should be departmental wise different. You will ask the motivation level for each employee.
And for this you can name the form "Self performance assessment". The employee will fil it and will give you back.

Hi Sanjay,

As rightly said by Minakshi, employees leave not because of organization but they leave becasue of supervisors. As it is been said "employees dont change the jobs they change the bosses".

As per my interpretation you want to know how to recognize the employees who wants to leave... for this

1. Obsever the employee behavior on indivial aswell as on team level. When an employee seem to be negative in his or her atitute, it means he/she is looking for change (reason can be anything).

2. Extra gossiping is another sing of the same.

3. Incrase in the complients from any specific employee.

4. Appllication for the salary hick in the middle of the aprraisal period. (ex. Apprisal are done in the month of March and in the month of June the employee wants his salary should be revised).

5. Increasing conflicts in the team members.

6. Increase in Absentism.

7. Not follwing the policies regarding working hrs., discipline.

I think this can provide you the path for your point. The main thing in all these points is Observation. You must be interactive with all the employees to get the insight of the issues going on. This will help you to recognize who is going to quite.

Hi, You should conduct exit interviews. Through this method you will be able to know the reasons from the employees for leaving the job from your company.

Hi Sanjay
I am 100% agreeing the views of Meenakshi
However, you can send a questionnaire on employee motivation for getting beedback and morale of the employees. By that also you can assess the pulse of employees and management attitude etc.
