Any one please help me to create a format for employee skill competency record, i am requesting to you all please send me a format document for the employee skill comp record.

Dear Kishore,
competency format you can divide it in to two one is required competency and another one is actual competency.
In required competency you can have the skills required by your organization (i.e skill requirement, Departments, Designation etc...)
In actual competency you have to keep the record whether the required competence have meet or not. any other additional skills apart form your requirements.
