Hi Dimpy,
We followed on-line application for Leave at our IT company and maintain leave records in excel sheets too.
Earlier our procedure was also through hard copy but this has been changed now.On-line helps in tracking details better & accountability of each person has been increased. Now the HR has also not to do alot of follow-up that we used to do earlier. On-line leaves are requested and get approved/rejected accordingly & the leave balance get updated.
As per my opinion, ISO implies that information has to secured & preserved, & readily available when required. So maintain proper leave records (Planned/Unplanned/comp-offs) of each employee whether the system is online or hard copy.

Hi Dimpy,
Keeping Leave application and details in hard copy is must in case of Manufacturing units, Factories, Some of the industries covered under Shops & Establishment Act of the State.
But in IT companies Leave are applied online through software, you only need to fill the applicable leave of a person for that year.. Automatically it gets balanced after application & approval from the concerned authority.
The leave details you may also maintain in an excel sheet...
For your reference, enclosed is one sample format of leave card in an excel.
