Can any one tell me what are the supporting documents to be submitted while applying for Provident fund withdrawal.
And i want to know is it necessary to mention the reason for fund withdrawal in withdrawal form???

Hi Deepti Along with the forms 19 and 10C, You can give cancelled cheque for the proof of bank account, but not mandatory.

It all depends on how many years you have worked with the company, If it's below 91/2 years then no document is required. Only Forms 19 and 10(C) needs to to filled up and submitted, duly signed by the authorized signatory. However, if you are working for more then 91/2 years then supporting documents like proof of age of your nominees are required for issuance of Scheme certificate by the P.F Authorities.
There is a separate column in both the forms which asks for the reason for separation from the company but you don't need to write the reason for fund withdrawal in these forms unless of course you are taking an advance from your P.F. balance in Form 31.

hi!
some times if it is delayed for PF withdrawal like 2 to 3 years or more, PF office may ask for a reason for delay to be addressed to Regional PF Commissioner and present bank account's statement along with form 19 & 10C. Actually, its recently happened with some of our colleagues.
