I a in process of preparing an leave card for the employees of the company.
Can anyone help me with an leave card format???
If anyone using an leave card for thier own company, can they ttach an copy of it?
Our leave policy is , we have annual 25 days leave, but we provide leaves to employees on earned leave basis ie 2 days per month,
how to show this on leave card ?
What columns should be included in leave card?
PLS help.

The Leave Card form usually include the following:
Name of Employee:
Designation:
Grade:
Basic pay:
Department:
Employee Code:
The following should be present in the tabular form:
1. Serial Number:
2. Type of Leave:
3: From
4. To:
5: Leave Due for the Year:
6. Accumulated Leave:
7: Adjustments : If Due Leave for a Year Exceeds 25
8: Total Accumulated Leave:
9: Special Leave: For Leaves like Maternity, Bandh etc
10. Remarks.
You can also look at Leave Card under Factories Act, 1948.

Hi Sara There should be provision for Sanctioning or Non Sanctioning. As mentioned by swastic pls refer factories act. regards JSN

Dear JSN,
Unless a leave is sanctioned it is not recorded in a Leave Card. Yes, some Companies do keep a column where Absent/LOP is recorded.
Some Companies also record Absent/LOP in Red ink under the head "Type of Leave".
