Accepting your resignation above referred you are relieved from service at the close of this (day) of (month) of (year). You may receive the final settlement amount from the Accounts Department. You are also requested to submit your EPF forms for further processing and onward submission to the EPFO for settlement/ disbursement of pension. With best wishes. Signature Copy to: 1.
