Dear Vivek,
The questionnaire is pretty good. For your general Idea, kindly organize youself what data is to be captured to study the perception of the employee. for eg.
1. The profile and role of the employee in the organization.
2. about the relationship with his peer, sub-ordinates and superior.
3. About the organization as a whole and departments. study the linkage between the department of which the employee belong and other departments and his perception about the functioning of the departments.
4. Try bringing in much more options than using yes/no type. you can use 5 rating scales also. You can use options like dissatisfied / highly dissatisfied / neutral / satisfied / highly satisfied.
5. club few questions into a single question and sub options so that when you convert the question into a chart, you will be easily able to relate / correlate things.
Make sure you have few questions which would enable you to do correlation study. for eg correlation between the employee work place and his perception about the role / responsibilities.
Take a pilot test of the questionnaire from your friends and may be you can get few suggestions from then since the questionnaire is objective for you and it is objective for them.....
if you have any other clarifications, be frank to revert back.....

thanks for the reply.
i am doing only one month training in the company and half the way is already gone. i will be in the company till 7th feb only. so it is a urgent need for me to complete the study. the questionnnaire designed was with the help of AGM HR.
now he wants to go more on it but due to time constraint for me, he has said to go on it finally.
i will send u the final questionniare prepared by me.
the topic is very interesting for me as it gives lot of scope for me.
now again a problem arised in front of me is to correlate the findings with the motivational theories testing and if possible some other things also.
i am not able to understand how to do it because i am doing it for the first time. i have studied motivational theories so for now according to this questionnaire only herzberg theory i think can be applied.
what else can be correlated is unknown to me also.
plz guide me. its very needed.

Dear Vivek,
Correlation is a tool which is used to relate between two factors.. Apart from herzberg theory, I hope you can also use Maslow's Need hierarchy theory being related to your questionnaire. There are few questions in your questionnaire which finds out the enviromental factors. why dont you relate this to the need hierarchy's Basic needs.
Find out the questions which you can relate to all the five needs... but make sure you do not deviate from the primary objective of the employee perception and biased towards the motivation theories completely. you can relate the experience with policies and systems question(q no 17) or with q no 21 or 29.
Free your mind and find which question can be related to the other question. I hope you will find many of the questions inter-related and you will find a solution.

thanks for the reply and giving me motivation to work on.
right now i am doing the survey but the worst part is that i am not able to interact with people because they are too busy in their work or either are only seeming to be busy because this a public private company and no one is eager to do work here.
now i am in a dilemma of how to get the response from these employees. and the worst part is my mentor has also been transfered to other department and now he no more remains hr head.
i am getting sick of this.

I hope this data collection stagewould be a tough situtation in a project. Only if the sample size is more, I think you can use much of the data interpretation tools. Try collecting as much as data required and is possible. You can request the people about the situation and request them to complete the questionnaire. If there is any training sessions happening in the company, you can ask the trainer's help and may be distribute the questionnaire and make it filled there.
