Yes you can visit esi website and download Form No. 1 i.e. declaration Tere is no form for permanent card. Once the employee is allotted ESI number along with temporary identification certificate (again it is part of declaration form no. 1) which is valid for 13 months from the date of appoitment.
Permanent Card: ESI local office will issue computerised card to the member from the date of appointment.
