I am attaching Training feedback form which i have developed for my organisation.I use this to give feedback of trainee's once my Induction training is over.(Would like to mention.My audience profile includes Business development executives and managers). I have tried to put SWOT analysis into this.
Would like to have valuable suggestion/feeback/inputs from members.

I am not a HR person, I uesd to be a manager years ago and we never had any formal training sessions. Hence, pardom me if Iask a number of questions rather than give a pat reply saying "thanks for posting, so useful".
What sort of Induction training are you giving?
How long does this training last?
Do you give hands on exercises (both individual and group)?
Do you give them tests at the end of induction training to assess product knowledge?
Would do you mean SWOT Analysis?
Who analyses the Strngths and Weakness and Opportunity and Threats?
What is this SWOT for? Is it for the trainee or for the organisation?
Do all the Skill and Competency areas have equal weight?
Why do Test 1 and Test 2 have different total points?
Have you used this to give feedback and, if so, have you asked for feedback on this from your executive trainees?
May be I am asking dumb questions. In which case, please ignore the posting.

Sorry for reverting late as i was on leave so cud not access net...
N let me first tell u that it was really nice to see ur queries..vl try n resolve ur queries one by one....
What sort of Induction training are you giving?
I take care of entire training reqirement of delhi ncr branch in my org.
So Induction training basically comprises of Introducing new joinees to
our organisation,brief idea of industry,our products and their KRA's and bit of Selling skills as required by target audience group.
How long does this training last?
Normally induction training goes for 7-8 days however refresher training ranges from 2-3 days session
Do you give hands on exercises (both individual and group)?
yeah i try to indclude more practical exercises n some mgmnt games in my sessions as it makes session more lively.
Do you give them tests at the end of induction training to assess product knowledge?
Not exactly at end of the sessio.Infact i have designed 2 tests for my induction batch.1st test is conducted on Day 3 and second test on Day 6th.

SWOT means stregth,weakness,opportunity and threats...Through this i try to analyse new joinee's personality n behaviour in 7 days session.
Who analyses the Strngths and Weakness and Opportunity and Threats?
What is this SWOT for? Is it for the trainee or for the organisation?
As mentioned earlier Trainer has to do SWOT analysis for new joinees.. and this feedback is forwarded to senior mgmnt /HR which is further kept in personal file of that employee.

Not exactly.it differs on basis of job profile of the audience
Why do Test 1 and Test 2 have different total points?
As nature of test is different n 2nd test carries more weightage as it chaecks product know of the candidate
