The "business plan" as the term suggests is for the business. Your post states that you would like to have a business plan for a "new vertical called patient support program". Is the patient support programme an independent business entity or one of the verticals of the existing organisation?
I wish you had given basic information like the nature of your primary business, who are your customers, their geographical location, etc. Similar information is required even for the new business vertical as well.
